The ulnar nerve at the elbow
Cubital tunnel syndrome involves irritation or compression of the ulnar nerve where it passes along the inside of the elbow. This nerve supplies feeling to the little finger and part of the ring finger. It also controls hand muscles used for coordinated finger movement and pinch. A problem at the elbow can therefore first appear as a hand symptom.
Bending the elbow changes the space and tension around the nerve. Leaning on the inner elbow can place direct pressure over it. Some people notice symptoms while holding a phone, driving, or sleeping with the elbow bent. Understanding the positions that bring on tingling helps guide both the examination and practical changes in daily activity.
Numbness and changes in dexterity
Tingling in the little and ring fingers can occur intermittently or become persistent. The inside of the elbow may ache. Difficulty spreading the fingers, using a key, or handling small objects can suggest an effect on the muscles supplied by the nerve. A change in handwriting or grip confidence is useful to report.
Visible muscle thinning in the hand deserves timely evaluation. Loss of strength can develop alongside a change in sensation, and advanced nerve dysfunction can leave lasting deficits. Symptoms may also arise from the neck or from compression at the wrist. An examination helps identify the location and whether another condition contributes to the pattern.
Examination and diagnostic testing
The clinician checks feeling, finger strength, and the response to positions that affect the ulnar nerve. The nerve may be examined while the elbow bends to see whether it moves out of its usual position. The neck, wrist, and other nerves may also be assessed to understand the full symptom pattern.
Nerve conduction studies and electromyography can help locate the problem and assess nerve function. Ultrasound may show the nerve's size, surrounding structures, or movement. The clinician selects tests according to the history and examination. Bring reports from prior testing and describe any previous elbow injury, surgery, or episodes of weakness. This information can change the interpretation of current findings.
Reducing pressure during daily activities
Changing elbow position can reduce irritation during rest and work. Avoid resting the inner elbow against a hard edge, and consider padding a desk or armrest. Adjust a phone or workstation so that the elbow can remain in a comfortable position. The goal is to limit the positions that reliably bring on symptoms.
Nighttime positioning or a prescribed brace can limit prolonged deep elbow bending during sleep. The support should fit comfortably and protect the skin. A therapist may recommend selected movements and changes in how you perform a task. Report persistent numbness or worsening strength even if a brace reduces nighttime tingling, because changes in nerve function can influence the treatment recommendation.
Surgery to relieve nerve compression
Surgery may be considered for continuing symptoms despite activity changes or for evidence of nerve dysfunction that calls for decompression. A release opens tight structures around the nerve. In selected circumstances, the nerve is moved to a different position in front of the elbow, a procedure called transposition.
The choice of operation depends on the nerve's stability, prior injury, and the anatomy found during assessment. Risks include infection, tenderness, numbness around the incision, nerve injury, and persistent or recurrent symptoms. A damaged nerve may recover incompletely even after pressure is relieved. Discuss the purpose of the procedure and which parts of your symptoms are expected to respond.
Recovery and follow-up
Recovery involves protecting the surgical area and restoring elbow and hand movement according to the procedure. Lifting limits and return to work depend on the operation and your job demands. Nerve recovery and wound healing follow different processes, so an incision that has healed does not establish that sensation or strength has fully recovered.
Hand therapy may help with movement, function, and adaptations while the nerve recovers. Ask how to monitor grip and finger coordination between appointments. Sudden weakness after an injury, severe swelling, or a cold or discolored hand requires urgent assessment. For gradual symptoms, early discussion of sleep disruption and lost dexterity helps establish a plan grounded in the tasks you need to regain.
Describe any loss of control when holding a mug or using a utensil. Concrete examples help the clinician assess the practical effect of a change in sensation or strength.
